Abstract:To determin if simple drainage operation is optimal to American Association for the Surgery of Trauma (AAST) grade III pancreatic trauma. Methodology: A series of 23 AAST grade III patients of blunt pancreatic trauma was retrospectively reviewed. A standardized operation of simple drainage was performed. Morbidity was compared between neck injured group and body/tail injured group. Results: The late-stage reoperation rate for unresolved pancreatic fistula was significantly lower in neck injury group than in body/tail injury group. Conclusions: Different management should be adopted according to the injured site within grade III trauma. Simple drainage surgery should be extended from grade IV/V injury to include neck injury of grade III trauma and distal pancreatectomy can be restricted to body or tail injury. | |||

Abstract:The aim of this study is to explore the optimal timing for surgical intervention of the partial injuried anterior cruciate ligament (ACL). Thirty-eight patients were divided into early surgery Group (n = 17) and delayed surgery Group (n = 21) based on the interval between injury and surgery, less than or beyond 3 weeks after injury. The minimum follow-up time was 2 years. The outcome measures used were the IKDC (International Knee Documentation Committee) scoring systems, Lysholm knee scoring scale, Tegner activity rating, range of motion (ROM) and the kneelax arthrometer. The preoperative associated lesions and side-to-side difference assessed by kneelax arthrometer postoperatively were significantly lower in the early surgery group than those in the delayed surgery group. No significant difference was observed in term of ROM, Lysholm, Tegner and IKDC scores 2 years postoperatively. The findings of this study indicate that early surgical reconstruction of the partial ruptured ACL could not result in arthrofibrosis, but prevent secondary loosening of the intact bundles and further meniscal and chondral injury. | |||

Abstract:BACKGROUND: Neurocysticercosis involving both brain and spinal cord is quite rare. Until now there are not many experience or standard treatments for that. CASE DESCRIPTION: A 23-year-old male patient presented the symptom of progressive numbness and weakness in the right lower limb. Magnetic resonance imaging (MRI) scans revealed the lesions involving both the spinal cord and the cerebral ventricle system. He underwent L3-5 laminectomy and the excision of extramedullary lesion. A postoperative course of albendazole was given for one month. Histopathological examination confirmed the cysticercosis. His symptom was completely released after operation. MRI showed lesions in the brain and spinal cord disappeared at the follow-up. CONCLUSIONS: Neurocysticercosis involving both the brain and spinal cord are rare and complicate. The combination of surgical and medical therapy could be the better choice for patients to achieve ideal outcome. | |||

Abstract:Genetic variations play an important role in tumor development and metastasis. Oligonucleotide microarrays were used to evaluate the genetic characteristics of tumor DNA in 30 HCC patients who were underwent LT. Recurrence-related single-nucleotide polymorphism were selected, and their prognostic value was assessed and validated in two independent cohorts of HCC patients, using pretransplant plasma circulating DNA. Prognostic significance was assessed by Kaplan-Meier survival estimates and log-rank tests. rs894151 and rs12438080 were significantly associated with recurrence (P=.003 and P=.004, respectively). Multivariate analyses demonstrated that the co-index of the 2 SNPs was an independent prognostic factor for recurrence (P=.040). Similar results were obtained in the third cohort (N=77). Furthermore, for HCC patients exceeding Milan criteria, the co-index was a prognostic factor for recurrence and survival (P<.001 and P=.002, respectively). Our study demonstrated first that genetic variations of rs894151 and rs12438080 in pretransplant plasma circulating DNA are promising prognostic markers for tumor recurrence in HCC patients undergoing LT. | |||

Abstract:Background Matrix metalloproteinases(MMPs) are considered to be essential for estimating the metastatic potential of tumor . Various studies examining the relationship between MMPs expression with the lymph-node involvement in Non-small cell lung cancer (NSCLC)patients have yielded inconclusive results. We try to explore whether there is an expressed value of MMPs in quantitating the risk of lymphatic metastasis of NSCLC. Patients and Methods The relation between MMPs and lymph node involvement (12 studies, n = 1875 patients )was examined. Fixed or random models are applied for estimation of the summarized odds ratio and 95% confidence intervals, including a test for homogeneity of the odds ratios(OR) of the studies. Results MMP-1, -2 was significantly associated with the presence of lymph node metastasis (MMP1: OR, 0.22; 95% CI [0.13-0.37]; P<0.00001 / MMP2: OR, 0.65; 95% CI [ 0.51-0.83]; P=0.0007). MMP-7 and -9 was not (MMP7: OR, 0.81; 95% CI [0.58-1.13]; P=0.21 / MMP9: OR, 0.92; 95% CI [ 0.48-1.78]; P=0.81). Conclusions Based on current data, expression of MMP-1 and -2 in the NSCLC patients might be associated with the involvement of lymphatic metastasis while MMP-7 and-9 might not be associated with the involvement. | |||

Abstract:OBJECTIVE: To model the microenvironment in vivo, and to co-culture the NSCs with normal hippocampal neuron and "epileptic neuron" for the observation of NSCs development. DESIGN: Repeated measurement and observation. METHODS: Rat hippocampal neurons were isolated and cultured, magnesium-free media treatment was applied to establish the model of "epileptic neuron". NSCs were cultured according to regular methods. After labeled by green fluorescence protein, NSCs were co-cultured with normal hippocampal neuron or "epileptic neuron" for 14 days, respectively. Patch clamp was used to record the electrophysiology of NSCs in co-culture; immunocytochemistry was used to demonstrate the expression of synaptophysin antibody of NSCs; patch clamp was also used to record the postsynaptic potential of the neurons differentiated from NSCs in magnesium-free medium. RESULTS: After NSCs was co-cultured with normal hippocampal neuron, 14 beats/5 minutes excitatory postsynaptic potential was recorded in 60% NSCs (6/10) by patch clamp; After co-culture with "epileptic neuron", 12 beats/5 minutes excitatory postsynaptic potential of NSCs was recorded. Immunocytochemistry revealed that 80% NSCs (12/15) was observed to express the synaptophysin in co-culture with normal neuron or "epileptic neuron". In magnesium-free medium, 14 beats/5 minutes excitatory postsynaptic potential with a duration of about 10 seconds was found in 60% neurons differentiated from NSCs (9/15), and no epileptic discharge was recorded. CONCLUSION: Rat hippocampal NSCs can form functional synapse in the co-culture with "epileptic neuron" in vitro. The possibility that NSCs develop into "epileptic neuron" is minimal. | |||
